We email you the test requisitions ordered by doctors. For lab test, it is recommended to book your appointment at lifeLabs, and take the lab requisition with yourself. For X-Ray and ultrasound, you need to contact radiology clinic, either the radiology clinic on top of the requisition form or any other radiology clinic and the report will be sent to us directly. Most radiology clinics accept walk-in for X-Ray. If you would like to review test results with a doctor, you can book virtual or in-person appointment.
